Aims & Activities
Mind & Motion Community CIC aims to improve wellbeing, independence, confidence and community participation for people experiencing mental health challenges, stroke, disability, neurological conditions and other barriers to participation.
We work alongside individuals to understand what matters to them, identify barriers to participation and develop practical, person-centred plans that help them reconnect with meaningful activities, social networks and their communities.
Our activities include:
Providing person-centred community support and wellbeing interventions.
Supporting people to reconnect with meaningful activities, hobbies, education, volunteering and employment where appropriate.
Helping individuals build confidence, independence and social connection.
Connecting people with relevant community groups, voluntary organisations and local services.
Working with professionals and partner organisations through referral pathways.
Providing telephone and community-based support where appropriate.
Monitoring progress and outcomes, including changes in meaningful activity, confidence, social connection, independence and community participation.
Developing partnerships with local health, social care, community and voluntary-sector organisations to improve access to support.
Our overall aim is to help people move beyond simply managing difficulties and towards living, participating, connecting and contributing within their communities.
